FormAssembly is a web form platform that enables businesses to collect and process data through configurable forms and integrations. Led by CEO and founder Cedric Savarese, the company is a Salesforce AppExchange partner and offers multiple features and integrations. FormAssembly serves thousands of customers, including Fortune 500 companies such as Amazon, Aetna and Volvo, across regulated verticals like financial services, healthcare, life sciences and higher education. In May 2019 the Bloomington, Ind.-based company raised $10 million in a Series A investment from Level Equity. As part of the deal, George McCulloch, a partner at Level Equity, will join FormAssembly's board of directors. The company said it will use the funds to continue expanding its remote team.
