The company deploys a hybrid care model that combines virtual care, mobile in-person care units, and in-home remote patient monitoring that uses cellular-based devices to capture biometric data. Its multidisciplinary teams provide virtual and on-the-ground services designed to augment local primary care physicians and hospitals. Homeward plans to accelerate deployments into key rural markets, build out its team, and coordinate entry into early markets ahead of service rollouts planned for summer 2022. The company intends to partner with regional Medicare Advantage plans to reach members and emphasizes value-based care and prevention through continuous monitoring.
