Patina Health has created a primary-care model tailored to adults 65+ that combines dedicated, multidisciplinary care teams with a purpose-built technology and analytics platform. The company plans to launch in-home and virtual primary care services in early ’22 in partnership with select Medicare Advantage health plans at no additional cost to participants. Its offering includes a mobile app to enable real-time information sharing and virtual visits across the care team, led by a primary care physician and supported by nurse practitioners, health champions and specialists (behavioral health, pharmacists, geriatricians, chronic-condition specialists and logistics specialists). The model emphasizes building trusted, ongoing relationships with older adults and their caregivers to improve satisfaction, clinical quality and lower total cost of care. Patina emerged from stealth with substantial venture backing to begin serving people and prepare to scale nationally, and is led by CEO Jack Stoddard.
