Project Well operates an online marketplace that connects health plans, at-risk provider groups, and their patients to food vendors offering nutritionally targeted, customized meals and integrated nutrition counseling. The service pairs members with registered dietitians and delivers personalized meals that account for medical, dietary, and cultural preferences. It currently works with two healthcare plans across a handful of Midwestern states and says its pilots produced high member engagement and satisfaction (for example, 19 of 22 respondents in a congestive heart failure pilot reported improvement or stability). The company targets vulnerable, often food-insecure plan members to address chronic-disease symptoms and related costs through diet. Project Well launched in early 2019 and plans to target additional health plans and expand into other markets as it scales.
