Radial Analytics develops Smart Placement, an AI-driven platform that delivers real-time, patient-individualized decision support to guide discharge planning and post-acute care placement. The platform applies machine learning and natural language processing and operationalizes "nudge theory" to present unobtrusive recommendations to clinicians and care teams. Smart Placement incorporates data from over 18 million patients and has processed more than 82 million patient encounters; it has been used daily by health plans and hospitals for more than three years. The product is deployed with payers and hospitals throughout New England and has demonstrated lower costs, improved outcomes, and better patient satisfaction by enabling more elders to return home with robust home-based services. The company plans to use new funding to grow its team and expand traction with Medicare Advantage plans and hospital systems engaged in value-based care delivery contracts. Financially, Radial Analytics announced a $3M financing led by Initialized Capital to support those growth plans.
