Unite Us provides the digital infrastructure to power an integrated health and social care ecosystem. It builds coordinated care networks enabling providers to send and receive secure electronic referrals, track each person's total health journey, and report on tangible outcomes across services. The company has signed hundreds of partners nationwide, including Kaiser Permanente, CyncHealth, the state of North Carolina, the Commonwealth of Virginia, Humana, CommonSpirit Health, CVS Aetna, the Cleveland Clinic, and United Ways. Unite Us began in 2013 serving the military-connected population and has expanded to serve Medicaid and Medicare beneficiaries, the homeless, justice-involved individuals, and people with behavioral health and substance-use challenges. The company is headquartered in New York City with offices in Raleigh and Portland. Financially, Unite Us announced a $35 million Series B and has raised $45.3 million in total funding to date. Management says the capital will fund growth, new products and innovations, and broader integration of social care into value-based payment models. The company plans to leverage its outcome data for predictive analytics and to drive payment reform and service integration across communities. Unite US operates UniteUS.com, a free resource network connecting current military service members, veterans and their families to local resources and opportunities. It provides organizations and communities with cloud-based organizational and care coordination software tools to communicate and manage services. The company powers NYC Serves, a coordinated network in New York City with over 40 public and private organizations and roughly 200 practitioners delivering coordinated care for the city's military population. Unite US reports upwards of 100,000+ users on its consumer platform and over 1,000 users accessing its software. Led by CEO Dan Brillman, the company recently closed a Series A to support its operations. Unite US is building a consumer‑facing platform that aggregates disparate veteran resources under one digital roof and uses interactive mapping and matching to connect users with organizations and peers. The company was founded by three members of the Armed Forces and is led by co‑founder and CEO Dan Brillman. Its product includes built‑in CRM tools for organizations to manage membership and collaborate, and a matchmaking/discovery approach that the team likens to Yelp and match.com rather than a professional network. The startup says roughly 10% of the product is focused on professional/job facilitation. Unite US is for‑profit and intends to partner with companies that value visibility and access among veterans. It is partly in stealth with a public launch planned for this fall and has raised initial outside capital to support development.
