Waymark hires, trains, and deploys local teams of community health workers, pharmacists, therapists and care coordinators to work directly with primary care practices at no cost to the practice, addressing care and social needs for Medicaid enrollees. Its proprietary Waymark Signal™ machine-learning technology identifies rising-risk patients and guides community teams toward evidence-based interventions, and is integrated into Waymark’s care management software that ingests data from ERs, primary care, social services and health plans.
