YourPath builds RecoveryWeaver, a communication-focused software platform that verifies insurance, facilitates virtual assessments and referrals, and aims to complete assessments within 24 hours of outreach. The platform connects with electronic medical records, attaches analytics and machine learning for predictive intelligence, and can link clients to support groups, social services, or child care. YourPath’s workflow starts with text-based outreach that routes people to a secure chat, consent forms and assessments; medical staff on the team can schedule care and write prescriptions when needed. Since launching in 2020, the company says it has helped more than 4,000 Minnesotans via virtual care and referrals, and in 2022 Blue Cross Blue Shield of Minnesota used the platform in its Blue Plus program and funded distribution of roughly 50 tablets at partner sites. Financially, the St. Paul startup recently closed a $2.1 million venture funding round and plans to raise at least $2 million more later this year to expand clinical and technical payroll. The company plans geographic expansion into Kentucky, Nebraska and Wisconsin as part of that growth.
