Welkin Health provides patient relationship management software for medical device companies, payers, providers, and disease management service providers. Its platform enables health care teams to work effectively and improve health outcomes. Led by CEO and co-founder Chase Hensel, the company has grown its team from 15 to 50 and has more than doubled its customer base since 2017. Welkin intends to use the new funds to continue expanding operations and its business reach. The company closed an $8m Series A with Thrive Capital in 2017 and has now raised a total of $30m. Welkin Health builds a digital patient relationship and disease-management platform, originally launched in 2013 as a digital monitoring system for diabetics. It evolved into a platform with robotic coaching tools and supports multiple healthcare verticals. The company currently supports 1,300 case managers working with 200,000 patients. Welkin plans to use new funding to build tools that let partners control and configure workflows within its system, reducing the need for internal configuration and integration work. It also intends to hire additional talent to accelerate partnerships with patient-centered digital health, life science, and medical device companies, and expects to announce new Fortune 100 partnerships in Q1 next year. Welkin joined Rock Health in 2014 and ran a major pilot with Humana’s Innovation Group in 2015. Welkin Health offers a care coordination platform that enables providers and health plans to manage daily outreach to hundreds of patients with chronic illness using telephone, SMS, and a smartphone app. The platform is designed to let care coordinators focus on patient relationships and streamline population-health workflows. Welkin has partnered with Humana’s Innovation Group to support telephonic diabetes outreach for elderly patients, with Humana testing programs in San Antonio and New Orleans that rely on the Welkin Coaching platform. The company plans to expand its platform beyond diabetes to include chronic-disease care for hypertension, COPD, and congestive heart failure. Welkin also intends to develop additional administrative tools for care organizations. The business emphasizes improving clinical and financial outcomes for providers and health plans through better patient engagement.
