The platform automates scheduling, improves patient engagement, and supports care navigators, case managers, and nursing teams to provide a concierge-like transition experience. Kouper says it can be fully deployed in as little as four weeks and has driven over a 50% increase in patient engagement and more than a 60% improvement in post-discharge and emergency department follow-up appointments among partners. The company partners with health systems and risk-bearing entities, and has pilot and expanded relationships with organizations including LifePoint, Mosaic Clinically Integrated Network/Henry Ford Health, and Ascension Saint Thomas.
