VitreosHealth provides an insights platform for population health management that delivers population risk models and predictive and prescriptive health insights. The platform generates predictive clinical risk scores, analyzes non-clinical markers and gaps-in-care, and produces prescriptive care recommendations. It surfaces to care managers, clinicians, administrators and C-suite which patients and members are likely to become high risk and predicts probability of hospitalization or escalation of chronic disease. Led by President and CEO Jay Reddy, the company is based in Plano, Texas and recently raised $5M in a funding round led by an affiliate of Kinderhook Industries, bringing total funding to more than $6.2M.
