The company partners with independent primary care practices to enable data-driven, holistic care and last-mile patient engagement. Vytalize supports roughly 130,000 senior patients through 280 primary care practices across 16 states and is responsible for $2 billion in medical spending. The business has grown its patient base 150% year-over-year and has begun contracting with regional and national Medicare Advantage plans and commercial plans. Last year the company acquired patient communication company MedPilot and is integrating additional downstream network partners, including hospitals, specialty networks, ancillary providers, and digital health companies. Management plans to use additional capital to continue investing in care delivery infrastructure, expand into new markets, and grow the team.
