It partners with primary care practices for more than 250,000 patients and delivers evidence-based recommendations at the point of care. The platform provides the right data at the right time in the clinical workflow to physicians and their staff to make practices more efficient and effective. Vytalize also offers financial incentives to practices that follow recommendations and drive better health outcomes. The company intends to use the new funding to accelerate its goal of advancing responsible value-based care for seniors by strengthening primary care practices. Vytalize Health offers an all-in-one, vertically integrated value-based care solution that combines a risk-bearing entity, a virtual and in-home clinic, and a broad technology platform. The company partners with independent primary care practices to enable data-driven, holistic care and last-mile patient engagement. Vytalize supports roughly 130,000 senior patients through 280 primary care practices across 16 states and is responsible for $2 billion in medical spending. The business has grown its patient base 150% year-over-year and has begun contracting with regional and national Medicare Advantage plans and commercial plans. Last year the company acquired patient communication company MedPilot and is integrating additional downstream network partners, including hospitals, specialty networks, ancillary providers, and digital health companies.
