Overview

Wellframe provides a patented Digital Care Management solution that delivers interactive clinical programs to members' smartphones and tablets and shares patient data with care teams via a care-team dashboard. The platform uses advanced algorithms to generate early intervention alerts and secure two-way messaging to sustain long-term relationships between patients and care teams. Health plans across commercial, Medicaid, Medicare and exchange lines use Wellframe to improve member experience and outcomes; the company says it more than doubled its health plan client base in the last year. Wellframe is accelerating the next phase of its product suite, Digital Health Advocate, which expands support beyond clinical needs to include administrative, financial and healthcare system navigation through a mobile app. The company plans to use the new funding to support growth and hiring in its Boston office and to further develop its digital offerings. Wellframe provides a mobile-enabled care management platform for health plans and providers. Its platform enables partners to leverage existing clinical resources to provide patients with ongoing guidance and support when and where they need it. The company takes a data-driven approach to evaluating performance and optimizing resource allocation for continuous improvement. Led by CEO Jacob Sattelmair, Wellframe intends to use the new capital to continue developing the platform and expand its business reach. In December 2017 it raised $15M in a Series B led by F-Prime Capital, bringing total funding to $25M to date. In conjunction with the round, F-Prime's Carl Byers will join Wellframe's board; existing investor DFJ participated in the financing. Wellframe combines mobile technology and artificial intelligence to amplify clinical relationships by engaging patients in personalized care plans. Its care plans cover areas such as mental health, diabetes and transplant medicine. The company has completed clinical trials with academic medical centers. Wellframe plans to announce a number of commercial partnerships in the coming months. Led by CEO and co-founder Jacob Sattelmair, the company uses machine learning to maximize patient engagement. Wellframe recently closed an $8.5M Series A financing to support its commercial expansion. Wellframe delivers an iOS and Android care-management application paired with a cloud-hosted dashboard for care managers. Its secure architecture renders personalized clinical protocols as daily to-do lists and is supported by an artificial intelligence engine. The company emphasizes human-centered process design to connect patients and amplify clinical care. Wellframe has completed clinical trials with academic medical centers and is a member of Rock Health’s fourth class. In February it rolled out its platform at South Shore Hospital to extend rehabilitation services for cardiology patients between visits. The company expects to announce additional customers among health plans and providers in the coming months.
