Locus Health (formerly Broad Axe Care Coordination) is a Charlottesville, VA-based provider of clinician-led, technology-enabled remote patient care management programs delivered on behalf of or in collaboration with providers. The company supports all Medicare readmission penalty conditions at the University of Virginia Health System, including acute myocardial infarction, heart failure, total joint replacement, chronic obstructive pulmonary disease, pneumonia, coronary artery bypass graft, and stroke. Led by CEO Kirby Farrell, Locus Health combines clinical teams with technology to manage remote patient care. On January 28, 2016, it completed a $4M Series B venture capital funding round. Backers in the round included the University of Virginia Health System and the Rothman Institute. The company plans to use the proceeds to continue expanding operations across the U.S. healthcare industry.
