Participated · Equity · Nov 2024
TailorMed provides an enterprise platform that manages the full lifecycle of patient support programs to reduce cost barriers and improve medication access and adherence. The company has built the nation’s largest Affordability Network, deployed across more than 800 hospitals, 1,300 clinics, and 650 pharmacies. TailorMed integrates directly with manufacturer assistance programs to automate patient support and streamline stakeholder workflows. Having reached an inflection point in its network evolution, the company is expanding into an end-to-end platform to tackle cost, access, and adherence across the medication continuum. The new financing will fund that expansion and broader efforts to create a seamless flow of data and resources across patients, providers, pharmacies, life sciences, and payers. TailorMed frames this work as addressing affordability at the point a prescription is written or dispensed to prevent delayed or skipped treatments. TailorMed Medical provides a technology platform that automates identification of financially at-risk patients and matches and enrolls them in financial resources such as co-pay assistance, replacement drug programs, government subsidies, community and state resources, disease-specific foundations, and programs addressing living expenses. Its platform is designed to reduce out-of-pocket responsibility for patients, helping to eliminate downstream financial hardship and avoidance of care. For health organizations, TailorMed shifts financial assistance from reactive matching or downstream collections to a proactive, point-of-care approach. The company serves physician practices, specialty clinics and health systems, and in 2021 expanded adoption into pharmacies nationwide. TailorMed said it will use the new funding to further expand its reach across health care with innovative financial solutions. Srulik Dvorsky is CEO and co-founder and the company is based in New York City.
- Axuall
Participated · Series B · Jun 2023
Axuall operates a workforce intelligence platform that aggregates clinician data through a national, real-time practitioner data network. Its Workforce Intelligence Network pulls clinician information from nearly 7,000 data sources and directly from physicians, advanced practice providers, and nurses via an integrated digital credential wallet with smart applications. The platform is used to reduce onboarding and enrollment time for healthcare systems, staffing firms, telehealth providers, and health plans while supplying data insights for network planning, analytics, and reporting. Axuall’s technology connects to the systems, vendors, and processes that customers already use. Led by co-founder and CEO Charlie Lougheed, the company has raised over $41M from more than two dozen healthcare organizations. It most recently announced an undisclosed Series B-1 that added Frist Cressey Ventures following a $20M Series B in May 2023. Axuall provides workforce intelligence powered by a national real-time practitioner data network, enabling healthcare systems, staffing firms, telehealth companies, and health plans to reduce onboarding and enrollment time while offering data insights for planning, analytics, and reporting. Led by CEO Charlie Lougheed, the company’s technology integrates with credentialing and recruiting workflows to streamline practitioner management. Axuall’s network today consists of over 6,800 primary data sources. The company recently raised $20M in a Series B to support growth. Axuall intends to use the funds to grow its product, sales, and marketing teams and to develop its data partnerships across its network. It also plans to accelerate API integration into planning, recruiting, credentialing, enrollment, CRM, and electronic medical record systems. Axuall operates a national digital professional identity network that enables clinicians, healthcare systems, staffing firms, telehealth groups, and health plans to share and manage authenticated provider credentials and qualifications in real time. Built on a provider-centric model, the network aims to eliminate onboarding delays, reduce physician burnout, and enable workforce intelligence across the healthcare industry. The solution was developed and piloted with University Hospitals, MedStar Health, and MetroHealth and launched commercially in late 2020. Axuall was formed in 2018 and initially raised $3 million of venture funding to develop the network. The company plans to use new capital to accelerate R&D, implementations, sales, and marketing as it grows its customer base of healthcare organizations. Management positions the product as addressing provider data delays and blind spots that drive billions in annual costs and care inefficiencies. Axuall operates a real-time digital network for verifying identity, credentials, and authenticity to eliminate waste, prevent fraud, and manage risk in healthcare, financial services, manufacturing, government, and other sectors. The network enables healthcare providers to request and receive digitally signed credentials from primary sources and securely share them with employers, health systems, and insurance plans. Axuall proactively monitors conditional events such as expirations and dependencies so practitioners can address issues as they arise, keeping credentials up-to-date and aligned with standards. Led by CEO Charlie Lougheed and based in Cleveland, OH, the company is preparing for commercial release. Axuall recently closed a $3M seed financing to support its near-term plans. The company intends to use the funds to hire additional software engineering and product personnel, run several large-scale pilots, develop its partner network, and complete R&D for commercial release.

Conversa Health provides an enterprise virtual care and communication platform that links providers’ EHRs and patient data to interactive digital care pathways and a clinical analytics engine to automate care management 24×7. Its platform and library of digital pathways are used by providers to manage patients across populations rather than one-off point solutions, improving pre- and post-visit care-plan adherence. During the COVID-19 pandemic the company’s customer count grew fourfold and its financial metrics increased sixfold as larger customers expanded use of the platform. To capitalize on recent momentum the company plans to hire more engineers and scale the platform. Company leadership believes remote and automated care will remain in demand after the pandemic and anticipates significant growth in virtual care delivery. Conversa Health offers an automated, chat-based virtual care and communication platform that triages patients, gathers patient data, and integrates with connected devices. The company positions its platform as the digital front door for health systems, aiming to scale personalized engagement and care. Conversa has launched Employee HealthCheck to screen workers before they return to work and has additional programs to screen for mental health concerns. The platform is being used by providers to check patients and loved ones before facility visits and to manage COVID-related workflows. Northwell Health has used Conversa to scale communications and care for thousands of COVID-19 patients with programs focused on lab results, quarantine and antibody tests. The company characterizes itself as, in many ways, a data company. Conversa is a San Francisco-based healthcare conversation platform that aims to improve doctor–patient communication. Roundtrip operates an online and mobile digital transportation marketplace that connects care coordinators, social workers, nurses and other transport requestors with non-emergency medical transportation providers. Its platform supports all levels of transport — medical cars, wheelchair vans, stretcher vehicles, ALS/BLS ambulances — across all payers and delivers trips when and where needed. Founded in 2016 and led by CEO and founder Mark Switaj, the company has built partnerships in over 40 states and is used across the healthcare landscape. Clients include health systems such as Lifespan Health and the National Institutes of Health, and payers including Community Health Plan of Washington and Contra Costa Health Plan. Roundtrip completed a $4M strategic funding round that brought total financing to date to $10.5M. The company plans to use the proceeds to geographically scale and specialize its sales, implementation and account-management model, expand its health-plan portfolio and technology integrations, and grow the Roundtrip Community marketplace. Roundtrip provides patient ride-ordering software that makes available a community of ride providers who complete non-emergency medical transports for healthcare personnel such as care coordinators, social workers and nurses. Its platform supports all levels of transport, including Lyft, facility-owned vehicles, volunteer drivers, medical cars, wheelchair vans, stretcher vehicles and ALS/BLS/SCT, and works with all payors. The company has built partnerships spanning over 20 states and operates from Philadelphia, PA and Richmond, VA. Founded in 2016, Roundtrip employs 26 people and is actively recruiting to support growth. The company intends to use its recent funding to enhance software capabilities and to attract sales, engineering and account management talent. Management plans to more than double the team as part of its growth strategy. RoundTrip operates a technology platform that connects patients to a slate of medical-transportation providers, including ambulances and specialty wheelchair-accessible vehicles, and handles billing for multiple payers within the same system. The company currently offers services in 15 states across the Northeast and plans to expand that footprint, with the CEO projecting a possible doubling of states over the next one to two years. RoundTrip employs the equivalent of 19 full-time staff at its WeWork Northern Liberties location and aims to grow that to 24 within three months; the recent capital raise is earmarked for expanding sales, marketing and engineering teams. Product development priorities include building additional features such as pharmacy stops after doctor appointments to create more patient value. The platform is positioned to integrate different vehicle types and payer sources to simplify recurring medical-transport needs, such as frequent chemotherapy trips. The company was founded to address transportation barriers that impede patient care, and management said the new funding will support both geographic expansion and technology build-out.
